**To guarantee a spot on the 2024 Roster, the following items must be received**

  1.  $100.00 Registration deposit made payable to “Pioneer Jr. Patriots”
  2. $50.00 Raffle Ticket Fundraiser
  3. All registration forms received in good order

Registration Form Checklist:

  • Registration Form
  • Emergency Medical Consent Form
  • SYF Code of Conduct Form, Signed/dated by parent and player
  • SYF Waiver Form
  • SYF Concussion Info Sheet, Signed/dated by parent and player
  • Physical Form (must be signed and dated by physician/medical facility after April 1st 2024)
  • Aeries/Student Data Information Form (Can be obtained from your child’s school, local school district. This is required for all new participants and all football players age 12 and up)

Additional information:

• All checks should be payable to Pioneer Jr. Patriots

• For football SYF is a NO minimum play rule, playtime is not guaranteed.

• A $25.00 administration fee will be assessed, in addition to any uniforms ordered, should your child drop prior to 7/30 of the current year.

• No refunds will be issued after 8/1 of the current year, no refunds beyond cost of customized uniform purchases.

• A $35.00 will be assessed for all returned check

• Parent hour checks will be returned if hours are completed in full and verified, 4 hour minimum per child, 8 hours max for 2 or more children.

• Registration for both football and cheer is due by 6/1/24

• Late Fee of $50 will be imposed on registration AFTER 6/1/24
